DEFINITION
The Admission Triage RN works under the general direction of the Referral/Admission Coordinator (RAC), Medical Director, and physician. The Admission Triage RN is responsible for processing incoming referrals from physicians, facilities, and patients and families consistent with the mission, vision, and goals of HWC, quality outcomes and regulatory standards. |

- Certified Hospice and Palliative Nursing preferred
- Minimum 2 years RN experience
- Valid driver’s license, car insurance and reliable car required
